Eligibility Determination Form

Our full time registration window has closed for the 34-25 school year. If you wish to use our home education program, please contact your zone school for more information.

You will need to schedule a meeting with the zoned school counselor to fill out the home education paperwork. After that is complete they will send the paperwork to the school choice office and withdraw your child.

The school choice office sends us the Notice of Intent and we process the registration and will send you an email.
